Welcome to Ervin Beauty. These Terms & Conditions explain the rules for using our website, booking appointments, purchasing products and services, and participating in the Ervin Beauty Membership.

Please review these terms before making a purchase or enrolling in membership. Specific booking, purchase, and membership terms will also be presented when applicable.

These terms do not replace medical evaluations, treatment-specific informed consent, privacy notices, or any rights you have under applicable law.

About Ervin Beauty

The Ervin Beauty website and membership program are operated by Ervin Cosmetic LLC, doing business as Ervin Beauty.

Ervin Beauty offers aesthetic services, including injectable treatments, skin treatments, and energy-based procedures.

Medical services are subject to evaluation and approval by the treating medical professional. The medical practice responsible for your care and the treating provider will be identified in your patient documentation and treatment-specific consent forms.

Membership purchases, appointment bookings, and promotional offers do not guarantee that a particular treatment will be appropriate for you.

Ervin Beauty Membership

Membership Fee and Minimum Commitment

The Ervin Beauty Membership starts with one upfront payment of $29.97 covering the first three months. This is the minimum membership commitment.

There are no additional membership charges during those first three months. Starting in month four, the membership renews at $9.99 per month until cancelled. All amounts are in US dollars.

After the initial three-month period, membership automatically renews monthly at $9.99 until cancelled.

Membership provides access to designated member prices and offers. It does not include complimentary treatments, products, consultations, or follow-up services unless an offer expressly states otherwise.

Membership fees are separate from all treatment and product charges.

Activation and Access to Benefits

Member pricing and eligible member offers become available after enrollment is completed, the membership agreement is signed, payment is authorized and the $29.97 initial payment is successfully processed.

You may use eligible offers during the initial three-month period. Your membership must remain active and paid when an eligible service is purchased and performed.

Receiving a member discount does not change the initial prepaid three-month membership term.

Automatic Monthly Billing

At enrollment, you separately authorize the $29.97 upfront membership payment and automatic monthly renewal of $9.99 beginning three calendar months after your start date. Renewal continues each month until cancelled. Your authorized payment method is charged by Ervin Cosmetic LLC, doing business as Ervin Beauty.

Your billing dates and cancellation instructions will be included in your enrollment confirmation.

If a billing date does not occur in a particular month, payment will be processed on the last day of that month.

Before enrolling, you will be shown the recurring price, billing frequency, minimum commitment, cancellation instructions, and any applicable introductory terms.

A Simple Billing Example

If your membership starts on October 10:

October 10: pay $29.97 for the first three months of membership.
November 10 and December 10: no additional membership payment is due.
January 10: the first $9.99 monthly renewal is charged, unless you have cancelled renewal before that date.
February 10 and each following monthly billing date: $9.99 is charged until cancelled.

Treatment, package and product charges are separate from this membership payment schedule. Your own dates will be shown in your enrollment confirmation.

How to Join

Contact our front desk at info@ervinbeauty.com or (818) 447-8202. Enrollment is currently assisted by our team through MangoMint; online self-service membership checkout is not available.
Review the membership agreement, initial payment, renewal date and cancellation instructions before enrolling. Sign the agreement and separately authorize the payment and recurring billing.
Our team processes the $29.97 initial membership payment and sets the first $9.99 monthly renewal for three calendar months after your start date. No extra $9.99 first-month membership charge applies.
Keep your confirmation showing your membership status, payment and next billing date. Ask the front desk to verify your member price and any benefit use before purchasing treatment.

Eligibility

Membership is available to individuals age 18 or older with an individual Ervin Beauty account.

Membership does not establish medical eligibility for treatment. Patient registration, medical history, evaluation, and informed consent may be required before receiving medical services.

Each membership is assigned to one person. Membership benefits may not be shared, transferred, resold, or used by another person.

Eligible Services and Offers

Member pricing applies only to services and products expressly identified as eligible on the membership page or in a specific member offer.

Services that are not listed as membership benefits remain subject to regular pricing.

Eligibility may depend on the treatment area, number of units, product quantity, number of sessions, or other conditions stated in the offer.

A member price for one treatment area does not automatically apply to additional areas or a more extensive treatment.

Member Offers and Benefit Limits

View the current member price list for eligible treatments, areas and quantities. Regular treatment prices remain separate.

TOX one-area offer: $79 includes 10 units for one treatment area and requires active membership. Additional units are $7.99 per unit. Your provider determines the appropriate area and number of units after assessment; 10 units may not be sufficient for every treatment plan.

Membership benefit limits: CO₂ and Morpheus8: up to 3 treatments each per 12 months. Endolift: 1 treatment per 12 months. These are membership limits; treatment timing and suitability are assessed individually.

PRP member package: 3 sessions for $299, paid upfront with membership enrollment. Any treatment area may be considered, with one tube per session. Your provider assesses suitability and the treatment plan; one tube does not guarantee coverage of an entire area or a particular result. The package fee is separate from membership: $299 plus the initial $29.97 membership payment totals $328.97 at enrollment. The membership then renews at $9.99 per month from month four until cancelled.

Morpheus8 member packages: face, 3 sessions for $1,050 paid upfront ($350 per session); face + neck, 3 sessions for $1,500 paid upfront ($500 per session). Individual member sessions remain $399 for face and $549 for face + neck. Membership fees are separate. The existing membership limit of up to 3 Morpheus8 treatments per 12 months still applies; a package does not add treatments beyond that limit. Treatment timing and suitability are assessed individually.

Any package-specific terms, including scheduling and the treatment of unused prepaid sessions, will be explained before purchase. The membership refund policy below applies to membership fees, not automatically to treatment-package payments.

Using Member Pricing

Please use the same account information for your membership and appointment booking so we can verify your benefits.

You may book eligible appointments online or through our front desk. Our team verifies active membership, the applicable member price and any benefit limits before payment. An online booking does not by itself enroll you in membership or confirm a member price.

Membership discounts cannot be applied retroactively to completed purchases or services.

Member pricing cannot be combined with other discounts, coupons, promotional offers, third-party vouchers, or special package prices unless the offer expressly permits it.

If more than one offer is available, the applicable price and offer conditions will be confirmed before payment.

Membership Is Not a Treatment Credit

Membership payments purchase access to membership benefits. They do not accumulate as treatment credits, deposits, savings, or stored funds.

Unused benefits do not roll over to future months unless expressly stated in a specific offer.

Membership has no cash redemption value and is not health insurance or a medical financing plan.

Membership Cancellation

You may request cancellation at any time. During the initial three-month term, cancellation takes effect at the end of that prepaid period and stops the next renewal, except where applicable law requires otherwise.

You do not need to wait until the initial three months have ended to submit your request. The $29.97 initial payment already covers that period; no second or third monthly membership installment is due.

After the initial commitment, cancellation takes effect at the end of the current paid monthly billing period. Submit your cancellation before the next renewal date to avoid another monthly renewal charge.

To request cancellation or get assistance, contact info@ervinbeauty.com or our front desk at (818) 447-8202. You may also use the cancellation method provided in your enrollment confirmation.

We will provide confirmation stating the cancellation effective date and whether a further renewal payment is scheduled. Benefits remain active through the end of the paid period, and no additional membership charge will be made after cancellation becomes effective. Keep that confirmation and contact us if the date or billing details do not match your request.

Cancelling an appointment, not using membership benefits, or opting out of marketing messages does not cancel your membership.

Membership Refunds

Except where required by law, membership payments are non-refundable and are not prorated once the applicable paid period has begun. This includes the initial prepaid three-month period and subsequent paid monthly periods. No refund or credit is provided solely because benefits were not used.

This policy does not prevent correction of duplicate payments, unauthorized charges, billing errors, or refunds required by law.

Failed Payments

An unsuccessful payment may result in suspension of membership benefits until the account is brought current. Our payment processor may retry the payment.

A failed payment does not itself cancel membership. Contact the front desk about your account or to request cancellation of future renewal.

If member pricing is unavailable because the membership is inactive or unpaid, the applicable treatment price will be explained before you agree to proceed.

We will not add an undisclosed late fee.

Changes to Membership

We may prospectively update eligible services, offers, and membership benefits. Material changes affecting existing members will be communicated as required by law.

The initial $29.97 membership payment covers the full first three months; that prepaid membership fee will not increase during the initial term.

Any later change to recurring fees will be communicated in advance within the legally required timeframe, with instructions for cancelling before the change takes effect.

Changes will not retroactively increase the price of completed purchases or remove benefits already purchased.

If we discontinue the membership program, future recurring billing will stop. Any prepaid period during which membership benefits are no longer available will be addressed through an appropriate refund or other remedy required by law.

Membership Questions

What does the first $29.97 payment cover?

It is the full membership payment for your first three months, equivalent to $9.99 per month. There is no additional membership charge during that prepaid period.

Are treatments included in the membership fee?

No. Membership provides access to eligible member prices. Treatments, products and prepaid packages are paid separately, unless a specific offer expressly says otherwise.

Can I ask to cancel before month four?

Yes. Submit your request during the initial term to stop renewal when the prepaid three months end. After that term, cancellation takes effect at the end of your current paid monthly period. Submit the request before the next renewal date.

Does joining mean I can receive any listed treatment?

No. A treating professional must assess medical suitability and approve the treatment plan. Membership does not replace informed consent, guarantee availability or guarantee a result. Results vary.

Appointments and Booking

All appointments are subject to provider availability, appointment confirmation, and the time required for the selected service.

Please book the correct service and disclose all treatment areas you would like assessed. Additional treatment areas or services may require a longer appointment or a separate booking.

Booking an appointment does not guarantee that treatment will be performed. Your provider may recommend a different treatment, postpone treatment, or decline a procedure based on clinical judgment.

Provider availability may change. If we need to reschedule your appointment, we will contact you to discuss available options.

Appointment Deposits

A deposit may be required to reserve an appointment.

The deposit amount, payment deadline, and applicable cancellation conditions will be disclosed before you pay.

Unless expressly stated otherwise, an appointment deposit is applied toward the total cost of your appointment and is not an additional treatment charge.

If a deposit is not paid by the stated deadline, the reserved appointment may be released.

Cancellations, Rescheduling, and Missed Appointments

Please notify us as soon as possible if you need to cancel or reschedule.

The cancellation deadline and any deposit forfeiture or missed-appointment charge will be clearly disclosed during booking and in your appointment confirmation.

A late cancellation, missed appointment, or late arrival that prevents treatment may result in loss of the booking deposit or a disclosed cancellation charge, subject to applicable law.

If Ervin Beauty cancels your appointment, you may choose to transfer your deposit to another appointment or receive a refund.

If the treating provider determines that treatment is medically inappropriate, we will explain how any deposit and separately disclosed consultation fee will be handled. A medical refusal will not automatically be treated as a missed appointment.

Arrival and Appointment Time

Please arrive with enough time to complete any required forms and preparation.

Late arrival may shorten the available appointment time or require rescheduling. We will not rush a procedure when doing so would compromise appropriate care.

Adding treatments during an appointment depends on available time, clinical suitability, and your consent to any additional cost.

Consultations and Photo Estimates

Photo-based estimates are preliminary. Photographs may not show all factors relevant to treatment planning.

Final recommendations, treatment areas, product quantities, and pricing are confirmed after an appropriate evaluation.

An in-person consultation fee may apply. The amount and any conditions for applying it toward same-day treatment will be disclosed before booking.

A consultation does not obligate you to purchase treatment.

Treatment Pricing

Treatment prices may vary based on the area treated, product selected, number of units or syringes, treatment complexity, and number of sessions.

Prices advertised as “starting at” represent a minimum price for the stated service, not a quote for every patient.

Your treatment plan and total price will be explained before you agree to proceed. Additional treatment will require your agreement to the additional cost.

Future prices may change. Confirmed purchases remain subject to the terms disclosed when purchased.

Injectables, laser treatments, and other medical aesthetic procedures require appropriate clinical evaluation and treatment-specific informed consent.

Please provide complete and accurate information about your medical history, medications, supplements, allergies, pregnancy or breastfeeding status, previous treatments, and any changes in your health.

Your provider will discuss relevant risks, expected benefits, alternatives, and limitations before treatment.

You may ask questions and decline treatment before it begins. Membership enrollment, payment, or agreement to these website terms is not consent to a medical procedure.

Results and Treatment Expectations

Results vary between individuals.

Outcomes may depend on anatomy, skin condition, medical history, product response, lifestyle, adherence to aftercare, and other factors.

No specific cosmetic result, duration of effect, or number of required treatments is guaranteed.

Before-and-after photographs and patient testimonials illustrate individual experiences. They do not promise that another patient will achieve the same result.

Some treatments require multiple sessions, maintenance, or additional treatment to achieve or maintain the desired improvement.

Aftercare and Follow-Up

Please follow the preparation and aftercare instructions provided for your treatment.

Contact the clinic promptly if you have an unexpected reaction, a concern about healing, or questions about your recovery.

Follow-up timing varies by procedure. A follow-up visit does not automatically include complimentary additional product, correction, or another treatment.

Any included follow-up or adjustment must be expressly identified in your treatment plan or offer.

For severe or rapidly worsening symptoms, seek urgent medical care. Do not wait for a response through social media, website chat, or email in an emergency. Call 911 when appropriate.

Payments and Financing

The amount due for your appointment will be disclosed before treatment.

Accepted payment methods and any available financing options will be communicated by the clinic.

Third-party financing is subject to the financing provider’s approval, terms, payment schedule, and policies.

Any lawful payment-related charges must be disclosed before you agree to the transaction. No additional fee will be added solely because it appears in these general terms.

Treatment Refunds

Except where required by law, completed services are not refundable solely because the result differs from a patient’s expectations.

Please contact us promptly about a treatment concern so the treating provider can assess it and explain appropriate next steps.

This policy does not waive your rights relating to billing errors, services not provided, professional negligence, or other rights protected by law.

Charges for products prepared specifically for your treatment, or cancellation after preparation has begun, will apply only when disclosed and agreed to in advance and permitted by law.

Refunds for unused packages or other prepaid services will be governed by the terms disclosed when purchased and applicable law.

Products, Packages, and Gift Cards

Product return conditions and package terms will be disclosed before purchase.

Package pricing applies only to the services and quantities included in that package. Any expiration, transfer restriction, or cancellation condition must be stated at purchase.

Gift cards are subject to their disclosed terms and applicable law, including any legally required protections concerning expiration and cash redemption.

Membership fees are not gift card purchases.

Promotional Offers and Any Future Rewards

Promotional offers apply only during the stated period and to the services or products identified in the offer.

Any limits on eligibility, redemption, quantities, or combination with other offers will be disclosed.

Membership does not automatically include loyalty points, referral payments, cashback, or redeemable credits.

If we introduce a rewards program, its separate terms will identify how rewards are earned, which purchases qualify, how they may be redeemed, and whether an expiration date applies.

Promotional rewards will not be treated as paid gift cards or patient deposits.

Privacy and Patient Information

Personal information is handled according to our applicable Privacy Policy and patient privacy notices.

Please avoid submitting sensitive medical information through public comments or unsecured social media messages.

Purchasing membership does not authorize us to publish your medical information or use your treatment photographs for advertising.

Marketing use of identifiable patient photographs or videos requires separate, appropriate authorization. Declining marketing permission does not affect your eligibility for care.

Text Messages and Email

We may send appointment confirmations, billing notices, membership updates, and other service-related communications using the contact details you provide, as permitted by law.

Promotional texts or emails are subject to applicable consent and opt-out requirements.

Consent to marketing communications is not required to purchase services or membership.

Opting out of promotional messages does not cancel your membership. Membership cancellation must be completed separately.

Website Information and Third-Party Services

Website content is provided for general educational and informational purposes. It is not a diagnosis or an individualized treatment recommendation.

We aim to keep service descriptions, prices, and offers accurate. If we identify an error affecting a booking or purchase, we will explain it before proceeding. You may decline a corrected offer and receive a refund for an unfulfilled purchase where applicable.

Our website may link to booking platforms, payment processors, financing companies, or other third parties. Those providers have their own terms and privacy policies.

Respectful Conduct and Account Security

Please keep your account information accurate and protect your login credentials.

We may restrict website or membership access in cases of fraud, account misuse, threats, harassment, or other unlawful conduct.

Any restriction will be handled consistently with applicable law, including patient care obligations and appropriate treatment of prepaid amounts.

Changes to These Terms

We may update these terms to reflect changes in our services, operations, or legal requirements.

The effective date will identify the current version.

Changes apply prospectively. Material changes affecting an existing subscription or purchase will be communicated as required by law, and additional consent will be obtained when required.

Posting an updated page does not itself authorize a new recurring charge or waive any existing patient or consumer rights.

Your Legal Rights

Nothing in these terms excludes or limits rights or remedies that cannot lawfully be excluded or limited.

These website terms do not release a treating professional from responsibility for professional negligence or replace any legally required patient consent.

If a provision is found unenforceable, the remaining provisions will continue to apply to the extent permitted by law.
